Eligibility:
- Have completed the Phase I deliverables (community health assessment, community action plan, and final report completed during year one)
- Be a tribe or tribal health agency within the state of California.
- Be seeking funding to support their tribe’s or tribal health program’s Community Action Plan.
- Have sufficient capacity and resources to complete the planned project activities within the project time frame.
Funding Amount: $5,000-$25,000
