Phase II: Implementation

Phase II of the ACORNS program is program implementation.  It is the Phase of the program where Tribal Community Teams, assembled in Phase I can implement their Community Action Plans. Phase II funding is available to tribes and tribal agencies who have completed Phase I.  If ACORNS is not able to fund a tribe or tribal agency a Phase I award, the tribal entity may still seek technical assistance from ACORNS to complete Phase I without funding.

Phase II programs should be highly centered around Policy, Systems and Environmental (PSE) changes to reduce chronic disease.
