(SCIHP) is a non-profit community outpatient health center established in 1971 serving the Sonoma County and South Coastal area of Mendocino County, with a mission to advance the health and well-being of American Indians living in our communities by providing access to quality, comprehensive and continuous health care, while respecting the traditional values of our community.
SCIHP has adopted the model Medical Home, also known as the patient centered medical home (PCMH), a team-based health care delivery model. SCIHP is a place where our patients are treated with respect, dignity, compassion, and enable strong and trusting relationships with providers and staff. Above all, the medical home is a model for achieving primary care excellence so that care is received in the right place, at the right time, and in the manner that best suits the patient’s needs. SCIHP is at the forefront of providing both integrative and preventative health care.
SCIHP works closely with our community providing advocacy on health care issues and ways to improve and maintain a quality of life addressing physical, mental, emotional, and spiritual needs.
SCIHP provides the following services: Medical, dental, behavioral health, pharmacy, nutrition, and transportation services. SCIHP’s satellite clinic, Manchester Point Arena (MPA) Rancheria is located on the beautiful pacific coastline. The MPA clinic provides limited services to tribal members.
